Guidelines: All participants must wear a mask (face covering) at all times when in the workshop All tools, safety rentals, and equipment will be sanitized prior to and after each workshop Hand sanitizer will be made available at each work station and should be used after touching common tools or equipment Work stations will be spaced at least six feet apart per for each group/activity Doors will be left open to ensure adequate ventilation (please dress appropriately for outside temperature) Participants who exhibit cold-like symptoms or COVID symptoms will not be admitted into the workshop—there will be no penalty for rescheduling due to illness COVID-19 Release Waiver: In consideration of receiving classroom instruction from E. J. Davis Aesthetics, LLC D.B.A. WorkWorkshop (the “Classes”), I do hereby forever release, waive, discharge, hold harmless and covenant not to sue E.J. Davis Aesthetics, LLC D.B.A. Work Workshop (the “Company”) and its member, employees, agents and representatives from any and all damages, injuries, losses, liability, claims, causes of action, litigation, or demands, including but not limited to those for personal injury, sickness, or death, as well as property damages and expenses, of any nature whatsoever, which may be incurred, directly or indirectly, now or in the future, in any way related to COVID-19, and in connection with my participation in the Classes. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the Classes and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I further acknowledge and understand that the risk of becoming exposed to or infected by COVID-19 during the Classes may result from the actions, omissions or negligence of myself and others. I understand that the Company has undertaken reasonable steps to lessen the risk of transmission of COVID-19, the Company is not responsible in any manner for any risks related to COVID-19 in connection with the Classes. I acknowledge and understand that participation in the Classes carries with it certain inherent risks related to COVD-19 transmission that cannot be eliminated regardless of the care taken to avoid such risks. Such inherent risks may include, but are not limited to: (1) the risk of coming into close contact with individuals or objects that may be carrying COVID-19; (2) the risk of transmitting or contracting COVID-19, directly or indirectly, to or from other individuals; and (3) injuries and complications ranging in severity from minor to catastrophic, including death, resulting directly or indirectly from COVID-19 or the treatment thereof. I hereby voluntarily accept and assume all risk of loss, personal injury, sickness, death, damage, and expense arising from such inherent risks. I do not suffer from any medical condition or disease that is contagious and/or might in any way hinder or prevent me from participating in the Classes, including, to my knowledge, COVID-19. This agreement shall be binding on my heirs, executors and administrators. I expressly agree that this agreement is intended to be as broad and inclusive as is permitted by applicable laws, and that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. This agreement may be executed, made and delivered electronically, and such electronic signature shall be considered as an original signature for all purposes and shall have the same force and effect as an original signature.

Workshop Safety Guidelines: All participants must wear eye protection at all times. Participants are encouraged to bring their own gear. We offer safety glasses and earplugs upon request. Participants are encouraged to dress appropriately for outside weather with layers to manage personal comfort Leather boots or durable shoes are highly recommended No open-toed shoes will be permitted in the workshop at any time…no exceptions. No consumption of alcohol will be allowed during any activities Use of power tools by participants is not permitted without assistance from an instructor All participants must wear workshop appropriate protective safety gear. Participants are encouraged to acquire their own protective gear ( eye protection, ear protection, welding masks, leather gloves, clothing cover/apron, etc. ) prior to arrival. Personal safety kits can be purchased at checkout on our site or rental kits can be rented for individual use All safety gear MUST be worn as instructed throughout each workshop or experience. Waiver of Liability and Hold Harmless Agreement: In consideration of receiving classroom instruction from E. J. Davis Aesthetics, LLC D.B.A. Work Workshop (the “Classes”), I do hereby forever release, waive, discharge, hold harmless and covenant not to sue E. J. Davis Aesthetics, LLC D.B.A. WorkWorkshop (the “Company”) and its member, employees, agents and representatives (collectively the “Released Parties”) from any and all damages, injuries, losses, liability, claims, causes of action, litigation, or demands, including but not limited to those for personal injury, sickness, or death, as well as property damages and expenses (including attorney’s fees), of any nature whatsoever, arising out of or related to any loss, damage or injury, including death, that may be sustained by me, or any of the property belonging to me, while participating in the Classes, or while in, on, or upon the premises where the Classes are being conducted, regardless of whether or not caused in whole or in part by the negligence or other fault of any of the Released Parties I am fully aware of the risks involved and hazards connection with the Classes and I hereby elect to voluntarily participate in the Classes with full knowledge that such activity may be hazardous to me and my property. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by me or any loss or damage to property owned by me, as a result of participating in the Classes, regardless of whether or not caused in whole or in part by the negligence or other fault of any of the Released Parties.
